Support Worker Older Adult CMHT at States of Jersey

An exciting opportunity has arisen for a support worker within the older adult community mental health team. This post holder will be expected to demonstrate an ability to support older people who are in need of secondary mental health services. The post holder will work with and support practitioners, the patient and their carers to assess needs and develop a holistic person centred support plan in the community.

This is a fixed term contract for a period of nine months. Driving licence and own vehicle required.
