Key Worker (Support Worker) at LIFELINE CARE GROUP (JERSEY) LTD

As a Key Worker, you will play a vital role in delivering consistent and person-centred support to individuals in our care. You’ll act as the main point of contact for your assigned service users, ensuring their care plans are followed, regularly reviewed, and reflect their personal goals, choices, and needs.

Responsibilities

Build trusting relationships with service users, families, and multidisciplinary teams.

Develop, implement, and review individualised care plans and risk assessments.

Support service users with daily living tasks, personal care, medication, and community participation.

Monitor and record progress, changes, and incidents accurately and promptly.

Promote independence, dignity, and respect.
