Job Description

As a Community Manager, you will play a key role in leading our community-based care operations. You’ll be responsible for overseeing the delivery of safe, high-quality care, supporting and developing our care teams, and maintaining strong relationships with service users, families, and partner organisations.

Key Responsibilities

Oversee day-to-day delivery of community care packages across Jersey.

Lead, motivate, and support care teams to provide outstanding person-centred care.

Conduct care reviews, spot checks, and quality audits to maintain service standards.

Act as the main point of contact for service users, families, and professionals.

Build partnerships with community organisations, health professionals, and local stakeholders.

Support recruitment, induction, and ongoing development of care staff.

Promote Lifeline Care Group’s values.
